It would be nice to have a feature were redlining your car or over revving can cause damage to the engine, causing it to eventually blow or lock up with oil splatter and smoke.

This already somewhat exists. Hitting the rev limiter will damage your engine with sim damage on. But 100% damage will not show smoke or oil leaks.
It does exist but the engine does not blow and lock up. It would be cool though.
The car is still drivable and the engine backfires and misfires a lot. But it does not lock up.
For Motorsport yes, horizon no.

It would be more realistic - and fun - to have all sorts of engine faults, and maybe even dashboard lights, but it should be separate to cosmetics
Also it might be better to fuel stops too, but I’m not sure if people would like that. Opinions?
Tho the introduction of this feature could be interesting to say the least, it would downplay or harm the gameplay, because the Forza franchise has always been recognized for its realistic yet arcade driving model, having a feature which ruins the flow of the “enjoyment” of the game misses the point of what the games are known for. While it can give a satisfying roleplay and realisitic experience, it can also be obnoxious for the player when its driving around in free roam and having its engine blown for the 635th time. And if the feature isnt functional and its cosmetic then (I think) the devs would put an insane time and effort over a very small detail
